1. All CCACs, SCs and The Unity Council will have a COMBINE MEETING ON THE 15 JANUARY 2011, SATURDAY 9:30AM

2. before attending the meeting, you will have to complete this online form. And please do read the front page of the form carefully before u proceed to fill in the form

However, the sec 4 and 5 for the last page u will not need to fill it in. you may just put N.A. as ur answer to the last page.

Lastly, please be punctual, latecommers will be dealt with okay?
